Demonstrates Strong Outcomes

As part of United Way of Racine County’s investment process, programs that receive funds must demonstrate the impact of the program on its clients beyond the numbers served.  Health Care Network Inc has chosen indicators which not only demonstrate improvement for the patient but also if successful have a positive effect on the community at large.

The first indicator demonstrates that people will significantly decrease Emergency Dept. (ED) usage if a viable alternative is available.  It is measured by the:

Percentage of new patients reporting a decrease in the use of the ED for non emergency health care needs

Actual findings

87% of new patients report a decrease in ED visits within 6 months of joining the program.

The second indicator focuses on controlling High Blood Pressure and Diabetes which left unchecked can lead to more serious health concerns and economic expense.  It is measured by the:

Percentage of patients treated on-site for high blood pressure or diabetes whose blood pressure reading and/or fasting blood sugar/glycohemoglobin have improved or are within normal limits.

Actual findings

71% of the patients met the indicator for success.

Finally with the addition of the on-site dental clinic the program has focused on education for improved home dental care.  It is measured by the:

Percentage of patients who demonstrate acceptable prophylactic care as evidenced by the oral health exam measuring calculus, pocket depth and inflammation of tissue.

Actual findings

85% of the patients met the indicator for success.
